This can help you share your health information with your doctor, contact a coach, and re-order your test strips and lancets when you are running low!

With the Diabetes Management program, members can set their own goals, understand their treatment plan, and receive assistance in the event of extreme blood glucose readings

This program provides support to keep your blood pressure under control. Enrollees will receive a cellular-connected blood pressure monitor and cuff. This makes it easy to capture your reading and share the information with your doctors. Hypertension Management will also help you with health “nudges” or suggestions based on your recent blood pressure trends. Coaches are certified chronic condition professionals that can help you improve your health outcome.
