When Am I Eligible?
Full-time employees working at least 30 hours per week are eligible for the Medical, Prescription, Flexible Spending Accounts, Dental, Vision, Employer Paid Life Insurance, Short Term Disability, Long Term Disability, Voluntary Life Insurance, Accident Injury Insurance, Critical Illness Insurance, Hospital Indemnity Insurance, Employee Assistance Program and Pet Insurance
Part-time employees working at least 20 hours per week are eligible for the Voluntary Life Insurance, Accident Injury Insurance, Critical Illness Insurance, Hospital Indemnity Insurance and Pet Insurance.
You are eligible to enroll in these benefits 1st of the month following 30 days of employment.
Are my Dependents Eligible?
Dependent Children are eligible to be covered under the medical and dental benefits until the end of the month to age 26 under the following guidelines:
Adult children shall include those who are a child of the Plan participant, whether or not they are:

Married or not married;

Live at home;
-
A dependent on the employee’s tax return; or,
-
A student.
Dependent children are eligible to be covered until the end of the month in which the dependent turns age 26.

How Do I Make Changes?
You are able to make changes to your benefits now for Open Enrollment, with changes becoming effective on April 1, 2025. You can enroll for the first time, waive coverage, change between plans or add/remove dependents.
At any other time throughout the year, you can only make benefit changes if you have a qualified change in status. Qualified changes in status include marriage/divorce, birth or adoption of a child, change in child’s dependent status, death of a dependent, change in residence (affecting coverage), or change in spouse’s benefits or employment status. If you experience a qualified change in status, please notify Human Resources within 30 days of the event. If you have a “Qualifying Event”, due to loss of eligibility under Medicaid or the Children’s Health Insurance Program (CHIP), you will have 60 days from the date of the loss of coverage under Medicaid/CHIP to notify Human Resources.
